by Peter S. Bergeron – April 6, 2026 as shared on Substack
Over the last several months, I have been sharing pieces of something I have been building for a long time.
The 12 fatal issues. The structural framework underneath them. The idea that small business failure is not random, not inevitable, and not the result of one catastrophic mistake. It is a pattern. And patterns, once you can read them, become something you can work with.
What I haven’t told you yet is where all of it has been heading.
The book and the platform launch together in mid-April. Not as two separate things, but as one. The Trapped Operator is the framework in full. 12fatalissues.com is where you step inside it firsthand. The book gives you the architecture. The platform gives you the environment to apply it to your own business, continuously, not as a one-time exercise but as an ongoing structural discipline.
Preorders are coming. More on that this week.
But first, let me bring it back to where we started.
The Pattern
Nobody tells you the truth about how small businesses actually fail.
They don’t blow up.
They erode. Slowly. Quietly. From the inside out.
And the owner is almost always the last one to see it, because they are too deep inside the machine to notice the cracks forming underneath them.
I have spent more than 30 years inside small businesses. Building them. Advising them. Watching them grow and watching them break. The pattern is always the same. It is never one catastrophic mistake. It is a collection of structural weaknesses, each one small enough to ignore, quietly compounding until the whole thing gives way.
That is what the Fatal Issues Framework™ exists to explain.

The Foundational Issues
At the Survival state, four issues dominate.
Inadequate capitalization. Poor cash flow management. Weak financial controls. Compliance risk.
These are not strategic problems. They are survival problems. A business that has not contained them is not in a position to build anything. The structural foundation that everything else depends on simply does not exist yet.
The Developmental Issues
If the business survives, it reaches Scale, and a different set of pressure points emerges.
Client overdependence. Customer neglect. Ineffective marketing. Poor market presence.
These issues don’t threaten the business’s existence the way the Foundational Issues do. They threaten its ability to grow without collapsing under the weight of that growth. A business that hasn’t contained them will hit a ceiling and wonder why nothing is working despite everything looking fine from the outside.
The Transitional Issues
If the business reaches Stability, the problems become existential in a different way.
Lack of strategic planning. Human resource problems. Failure to evolve. Ineffective management.
By this point the owner has usually survived enough to believe the hard part is behind them. It isn’t. The Transitional Issues are the ones that determine whether the business can outlast the founder. They are the difference between building a transferable asset and building something that only works because one specific person is at the center of it.
Containment
Most frameworks tell you to fix everything. That is not how structural pressure works.
You do not have the time, capital, or bandwidth to eliminate every weakness at once. The Fatal Issues Framework™ is built on a different premise: identify the issues creating the most structural pressure at your current operational state, and contain them. Prevent them from spreading. Move forward from a contained position rather than an exposed one.
Not every issue demands your attention today. But the ones inside your current state do.

What Is Coming
In mid-April, the book and the platform launch together.
The Fatal Snapshot™ is a six-question structural assessment that identifies which of eight business profiles describes where your business actually is right now, from At-Risk to Fully Owned Business.
The Fatal-48™ is the comprehensive structural diagnostic. Forty-eight questions. A heat map of where the structural pressure is concentrating inside your business and which of the three containment systems needs the most attention.
The Trapped Operator™ walks you through the full framework, the profiles, all twelve issues, and the containment disciplines for each one.
They are designed to work together. You read the framework. You take the diagnostic. You come back with your own results in hand.
If any of what I have been sharing over the last several months has felt familiar, that recognition is exactly where this work begins.
Preorders are coming this week. Stay close.
